Micropigmentation Consent Form

Micropigmentation - Microblading, Ombre/ Powder & Nano Brows, Lip Blush, Dark Lip Neutralisation, Paramedical Tattoo, Scar Camouflage & Scalp Micropigmentation (SMP)

PERMANENT RESTRICTIONS TO HAVING MICROPIGMENTATION:

The following facial procedures must be avoided 4 weeks before a PMU procedure.

No BOTOX / Fillers

No Chemical Peels / Pumpkin peel / Enzyme peel /Glycolic etc

Electrolysis hair removal / wart, skin tag removal

Microdermabrasion Treatments

Laser / IPL Facials) ablative or non- ablative

Laser Hair Removal

Do not tan for 2-4 weeks before your PMU treatment

I have read and understood the above

Risks associated with permanent makeup/ micropigmentation

Infection and bleeding

Scarring

Allergic reactions

Skin discoloration, such as hypopigmentation and hyperpigmentation

Temporary changes in skin texture

I have read and understood the above

2 days prior to treatment, I have not taken the following:

Anti- inflammatory meds such as Ibuprofen

Alcohol

Aspirin

The client understands that receiving Botox injections can impact the appearance of their permanent makeup (PMU) on the brows and SMP. As Botox temporarily alters the position and movement of facial muscles, it may cause the brows to appear differently while the Botox is active. Once the effects of Botox wear off, the muscle activity returns, which can lead to changes in the brow position and potentially affect the symmetry or placement of the PMU.

The client acknowledges that these changes are a normal result of Botox wearing off and are not a reflection of the quality or outcome of the PMU procedure.

I understand the above statements, therefore I will book my PMU 1 month after my botox appointment.

If my botox wears off and effects the results of my PMU. I do not hold my PMU artist responsible.
